CalAIM Enhanced Care Management (ECM)

Enhanced Care Management (ECM) is a comprehensive service for Medi-Cal members with complex medical, behavioral health, and social needs.

ECM services include:

• Outreach, engagement, and enrollment
• Comprehensive medical, behavioral health, housing, and social assessments
• Individualized care plan development
• Ongoing care coordination across providers and systems
• Coordination with primary care, specialists, hospitals, and clinics
• Medication follow-up and appointment support
• Transitions of care following hospital or institutional discharge
• Crisis prevention and stabilization support
• Health education and self-management coaching
• Continuous reassessment and documentation
