Our Community Health Worker program provides direct, hands-on support to individuals navigating healthcare, housing, and social service systems. CHWs serve as trusted connectors between individuals and providers.

CHW services include:

• Community outreach and engagement
• Medi-Cal eligibility screening and enrollment assistance
• Health plan navigation and education
• Appointment scheduling and coordination
• Care coordination with medical, behavioral health, and social service providers
• Health education and chronic condition support
• Public benefits navigation (SSI/SSDI, CalFresh, General Relief)
• Ongoing follow-up, progress tracking, and documentation
