Helping Hands for Creative Support Inc

Individuals may qualify for services if they meet one or more of the following criteria:

• Currently enrolled in Medi-Cal with an active managed care health plan
• Eligible for Medi-Cal and require assistance with enrollment
• Experiencing homelessness, housing instability, or risk of displacement
• Managing chronic medical conditions, mental health disorders, or substance use disorders
• Transitioning from hospitalization, institutional care, or crisis settings
• Referred by a health plan, hospital, provider, community organization, or self-referred
